To achieve results that aren't of that nature, the base articles of clothing must be a size larger than one would wear directly off the rack. That way, one's differences from average proportions can be accounted for in a way that does not, in your words, "choke" the wearer. Indeed, this is precisely what I've done with the summer variations on my suit.
