Apparently it's made up of three worlds: Avalar, where people tend to land; the Dragon Realms, and the Forgotten Realms. Each of those three are split further into homeworlds, which have realms that can be accessed from each one.

And apparently everyone gets a dragonfly that's... in some sort of way, tied to their physical well-being. It changes colors if a person gets sick or injured. [There's more to it than that, but...]
