[head cannon explanation here: I believe that Poniko and Mado knew eachother as kids,(when Poniko was a man) and Mado was the first person Poniko told, Mado just gave no tosses, because Poniko is Poniko right? and that's where the lack of caring about gender for Mado Comes from. then Poniko moved away and in that time either more embraced or had the operation and chose to start a new life thus ignoring Mado
